Getting Started: How to Install and Set Up SmartForm

Before You Start


Log in at portal.simplecheck.ai with the email used for your SmartForm account. If this is your first login, complete the password setup shown on screen.


GoHighLevel now uses OAuth. Do not copy/paste a legacy GHL API key for a new setup.

First-Login Onboarding


On a new account, the portal may first show:


  1. Business — add your business/DBA name.
  2. Calendar — connect Google Calendar or GoHighLevel Calendar.
  3. Funnels — optional ClickFunnels step when enabled for your account.
  4. CRM — connect a recommended CRM or continue and connect it later.


After onboarding, continue to the widget editor and complete the widget-specific steps below.



1. Connect Your Account-Level Tools


Open Connections from the left sidebar.


  1. Use Available to find the provider.
  2. Connect the calendar and CRM accounts you need.
  3. Complete the provider's OAuth window.
  4. Confirm the provider appears under Connected.


For GoHighLevel, one OAuth connection can support both lead sync and GHL calendar bookings. For HubSpot, Pipedrive, and ClickFunnels, follow the current connection flow shown in the portal.

2. Create or Open a Widget


Open Widgets and create a new widget or select an existing one.


The setup wizard uses these steps:


  1. Type — choose the widget/form experience.
  2. Team — assign the calendars and team members used by a booking widget (lead-only widgets can skip this calendar step).
  3. Design — configure content, branding, form fields, and preview.
  4. Routing — choose what qualified and not-qualified leads see next.
  5. Launch — preview, copy the embed code, and open platform-specific install guides.



3. Assign Calendars and Team Members


In Team:


  1. Select the Google Calendar or GoHighLevel members used by this widget.
  2. Add an optional personal Zoom, Google Meet, or Microsoft Teams link for each member when needed.
  3. For Smart Routing, use Calendar Teams for Smart RoutingAdd Team, name the team, and select its members.


Only selected members expose availability for that widget. Different widgets can use different teams.



4. Design the Form


In Design:


  • Add the widget/customer-facing title
  • Upload the logo
  • Configure form fields and supporting copy
  • Choose the available display style
  • Review the live preview


Keep the required name, email, and phone fields needed by your workflow/X-Ray setup.



5. Configure Routing


In Routing, choose the result for qualified and not-qualified leads. Available actions can include:


  • Show a calendar (all assigned members or a selected Calendar Team)
  • Redirect to a URL
  • Show a message
  • Run configured CRM actions


Smart Routing is included with SmartForm; there is no separate Smart Routing subscription step. X-Ray Vision is required for credit-data rules and is automatically enabled/locked while Smart Routing is active.



6. Review Widget Integrations


In the widget's integration panel:


  • CRM — verify provider connections and select a ClickFunnels workspace when needed
  • Leads — enable/verify Smart Capture and email validation
  • Webhooks — add custom Lead Created or Booking Created endpoints
  • X-Ray — activate/verify X-Ray Vision



7. Launch and Embed


Open Launch (or Preview & Embed in the full editor):


  1. Click View Preview and complete the form once.
  2. Copy the entire Embed Code block exactly as shown.
  3. Paste it into your website/funnel's HTML/embed element.
  4. Publish the page and test the live page—not only the page-builder editor.



8. Test End to End


Use a real email/phone you control and verify:


  1. The form submits successfully.
  2. Smart Routing chooses the expected result.
  3. A booking appears on the intended calendar/team.
  4. The lead appears in Bookings & Leads and each connected CRM.
  5. Smart Capture appears under Smart Capture if you intentionally abandon a test form after entering contact information.


Troubleshooting


A provider says connected but the widget does not use it

Account-level connections and widget-level assignments are separate. Re-open the widget's Team or CRM step and select the calendar/workspace needed by that widget.


The OAuth popup does not open

Allow popups for portal.simplecheck.ai, then retry.


The embedded widget does not appear

Publish the host page, ensure the embed container has no restrictive fixed height/overflow, and copy the code fresh from Launch.


If setup still fails, contact support in the portal or email support@simplecheck.ai with the widget name and failing step.
