Getting Started: How to Install and Set Up SmartForm

🛠️ How do I install and set up SmartForm?

Summary:

Learn how to install and configure SmartForm with GoHighLevel or Zapier, connect your calendar and Zoom, and access your SmartForm setup portal and walkthrough video.



SmartForm currently integrates directly with GoHighLevel (GHL). If you’re using another CRM, you can push contact data via Zapier. We also support Google Calendar integration and allow hard-coded Zoom links for booking automation.


To install and configure SmartForm:


  1. Watch the setup video



  1. Log into the SmartForm Portal

👉 Log In Here

Use the same email you purchased with.

Click Create Password, then Sign In.


  1. Edit your booking widget

Customize the look and feel:

  • Name your widget
  • Upload your logo
  • Toggle social proof and limited-time badges


  1. Connect your Google Calendar

In Integrations, click Connect under Google Calendar.

⚠️ If Google warns about permissions, click:

Advanced → Go to SimpleCheck Portal → Continue


  1. Connect your CRM

Every lead and booking is stored in your SmartForm dashboard by default.

Using GoHighLevel?

  • Grab your API key (GHL → Settings → API Key)
  • Paste it into the GHL field inside SmartForm

More CRMs will appear automatically in the Integrations tab as they go live.


  1. Set your availability

Go to the Availability tab and select the days and times you want to accept bookings.


  1. (Optional) Turn on Smart Routing

Smart Routing lets you redirect leads based on their credit data.


  • Enable Smart Routing
  • Complete the billing setup
  • Create routing rules like:

 → Score < 650 → Nurture funnel

 → Score ≥ 651 → High-ticket page


  1. Customize form fields

Default fields: First name, Last name, Email, Phone

➕ Add any extra questions your sales process needs.


  1. Embed your widget

Copy the embed code from your SmartForm portal and paste it into the landing page or funnel where you want it to appear.


  1. Test everything
  • Submit the form like a lead
  • Make sure routing works
  • Confirm booking appears in Google Calendar
  • Check lead shows up in GHL (if connected)
